Integrated Rehabilitation Therapy
The primary objective of rehabilitation therapy in the skilled nursing environment is to enable individuals to regain strength, functionality and independence following illness, injury or age-related infirmities. Rehabilitation therapy may include one or more disciplines including physical therapy, occupational therapy and speech language therapy. These disciplines are integrated and coordinated with skilled nursing, directed activities and social services professionals to deliver an effective plan of care that restores strength, functionality and independence. Examples of rehabilitation therapy include:
- Strengthening and conditioning exercises
- Range-of-motion and flexibility exercises
- Wound care
- Splinting and contracture management
- Activities for Daily Living (ADL) training
- Adaptive dressing, bathing, grooming, dining, etc.
- Gait (e.g. walking & ambulation) training
- Transfer (e.g. standing to/from bed or chair) training
- Bed or chair mobility and positioning
- Restraint reduction
- Assistive device training
- Dysphagia (swallowing) treatment often due to dementia, stroke or trauma
- Aphasia/Cognitive Impairment Treatment often due to dementia, stroke or trauma

Physical Therapy
Physical therapists focus on building strength, as well as improving range-of-motion and flexibility. This is accomplished through assisted and unassisted physical exercises, training and manipulation of the individual. Such exercises may include gait training and transfers. Licensed physical therapists may also provide wound care and wound care training.
Occupational Therapy
Occupational therapists are focused on helping individuals regain normal functionality for independence and, where necessary, adaptive and/or assisted functionality. This is accomplished through assisted and unassisted physical exercises and activities, utilizing adaptive techniques and devices and training of the individual and their caregiver(s).
Speech Language Therapy
Speech language therapists are focused on managing and/or mitigating risks of swallowing and communication (e.g. speech, hearing and cognition) for individuals who are impaired due to dementia, stroke or trauma. This is accomplished through modifications in diet to avoid aspiration (e.g. thickened liquids), utilizing adaptive communication techniques and devices and training of the individual and their caregiver(s).
